All students interested in private music lessons during the school year should complete the form below. You will be contacted by a private lesson teacher who will arrange for an interview with you during the first week of school.

Health Office
All new students must undergo a physical examination in June or July prior to the start of school. Please complete and submit the form below to nurse@bfacademy.de. The first two pages are to be completed by the family and the third page by the doctor or nurse practitioner. This will also meet the physical exam requirement for co-curricular sports participation.
Parents must complete the BFA Medical Release form annually for each student enrolled at BFA. The form is currently closed. It will open for the 2024-25 school year on July 1, 2024.
The Medication Authorization form must be completed by a physician or nurse practitioner for each prescription medication a student will be bringing to BFA. A form is not needed for over-the-counter medications, vitamins, or supplements. Please email the completed form to nurse@bfacademy.de.
The Vaccination Consent Form is required for students under the care of the Health Office or through Health Office Vaccination Clinics. Please email the completed form to nurse@bfacademy.de.
Athletics
Tryouts for fall sports (Volleyball, Cross Country, and Golf) begin on the first day of classes. Any students interested in playing any of these sports should come prepared with a change of clothes and running shoes for practice.
All student athletes in grades 9-12 participating in co-curricular sports and grade 8 students in cross-country must complete a physical examination annually in June or July prior to the start of school. Students will not be allowed to participate in a sport after tryouts if they do not have a physical on file that is less than one year old.
