Parents are responsible for making all travel arrangements for their children. Travel plans for dorm students must be submitted to BFA Residence Life at least one month before the start/end of the school year and each break outlined in the School Calendar. These plans must include all schedule information for each step of the student’s journey. The Residence Life office will also be in contact with families in the lead up to each of these travel times. For more details regarding travel information and expectations, please review the ResLife Parent Handbook.
BFA will provide transportation for students to and from Basel Airport (BSL), and Basel Badischer Bahnhof and Basel SBB train stations. Dorm assignments are finalized in July. Students and parents will be notified individually once their placement in a dorm is finalized. Room assignments are not released prior to student arrival.
Below is a list of links and important information regarding Health forms that are required for new students and/or annually for all students.
All medical forms are DUE by August 1st.
Medical Release Form
Parents must complete this form annually for each child enrolled at BFA. If more than one child is enrolled at BFA, a separate form must be completed for each child.
Medication Authorization
The Medication Authorization form must be completed by a physician or nurse practitioner for each prescription medication your child will be bringing to BFA. You do not need to fill out a form for over the counter medications, vitamins, or supplements. Please email the completed form to nurse@bfacademy.de.
Vaccination Consent
This form is required if your child will be receiving vaccinations while under our care or through our Health Office vaccination clinics throughout the year. Please email the completed form to nurse@bfacademy.de.
Physicals
All new students must complete a physical examination within one to two months prior to the start of school. The first two pages are to be completed by the family and the third page by the doctor or nurse practitioner. This will also meet the physical exam requirement for co-curricular sports participation.
All student athletes in Grades 9-12 participating in co-curricular sports and Grade 8 students in cross- country must complete a physical examination annually (at least two weeks prior to the beginning of the sports season). Families are asked to complete this in the summer, prior to arrival at BFA, so that a student’s sports participation is covered for the entire year. Students will not be allowed to participate in a sport after tryouts if they do not have a physical on file that is less than a year old.
Please email the completed form to nurse@bfacademy.de.
Orthodontic Care
If your child will need orthodontic care, please contact the Health Office (nurse@bfacademy.de). The Health Office will respond as soon as possible.
Athletics
All athletes need to complete a Physical Evaluation form each year in order to participate. Detailed information and a link to the form can be found on the Health tab. Tryouts for Fall sports (Volleyball and Cross Country) begin on the first day of classes. Any interested students should come prepared with a change of clothes and running shoes for practice.
Performing Arts
All students interested in private music lessons during the school year should fill out the form below. From there you will be contacted by a private lesson teacher and make arrangements for an interview during the first week of school. Students who are interested in renting an instrument for the school year should contact tim.cocking@bfacademy.de.
Middle School Co-Curricular Activities
MS Students take part in a number of activities throughout the year. Parents of students in grades 5-8 must complete this form by August 1st.
